Job summary

The ideal candidate is patient, reliable, and works well as part of a team, with 1 year of long-term care experience and NY CNA certification. Per diem roles may include variable shifts and on-call coverage.

Qualifications

• Current NY CNA certification
• 1 year long-term care experience required
• High school diploma or equivalent preferred

Responsibilities

• Provide assistance with activities of daily living including bathing, dressing, grooming, toileting, and feeding.
• Take and record vital signs and other patient information as directed.
• Assist with transfers, repositioning, ambulation, and mobility.
• Maintain a clean, safe, and comfortable environment for residents/patients.
• Observe and report changes in residents' condition to the nursing staff.
• Assist with meals and monitor food and fluid intake as needed.
• Provide compassionate emotional and social support.
• Follow all facility policies, procedures, and infection-control guidelines.
• Accurately document care provided.
• Work collaboratively with nurses and other healthcare professionals.
