Job summary

Autumn Hospice is seeking a Certified Nursing Assistant (CNA) to join the South Team and provide direct, compassionate end-of-life care. You will assist with ADLs, mobility, meals, and monitoring, supporting patient comfort and dignity.

Requirements include CNA certification, willingness to travel, a valid driver's license, and reliable transportation. Hospice or home health experience is preferred but not mandatory.

Qualifications

• Must possess a current state CNA certification.
• Hospice or home health experience preferred.
• 1 year of CNA experience preferred; not required but helpful.
• Willingness to travel across the assigned territory; valid driver license required.

Responsibilities

• Provide or assist with ADLs including personal hygiene, grooming, mouth care, skin care, and nail care.
• Assist with toileting, positioning, and transfers; frequent turning of bed-bound patients.
• Assist with prescribed exercises and ROM as instructed by Hospice Nurse.
• Perform light housekeeping in patient room and laundry as needed.
• Assist with meals and monitor patients during feeding.
• Take and report vital signs (temperature, pulse, respirations) as directed by the Hospice Nurse.
• Report changes in patient condition to Hospice Nurse and document appropriately.
• Assist or remind patients to take medications in compliance with regulations.
• Deliver compassionate care to patients and families, maintaining dignity and comfort.
• Communicate effectively with the Hospice Nurse and care team; uphold HIPAA and policies.
• Maintain patient confidentiality and compliance with hospice policies.

The Certified Nursing Assistant (CNA) will be responsible for, but not limited to:

Responsibilities

• Provides or assists with activities of daily living (ADLs), including personal hygiene, grooming, mouth care, skin care, and nail care.

• Provides assistance with toileting, positioning, and transfers, including frequent turning and positioning of bed-bound patients.

• Assists with prescribed exercises and range-of-motion (ROM) activities that the patient and Hospice Aide have been instructed on by the Hospice Nurse.

• Assists with light housekeeping of the patient's personal living area or room, including changing bed linens and patient laundry as needed.

• Feeds and assists patients with meals as appropriate.

• Obtains temperature, pulse, and respirations as directed and supervised by the Hospice Nurse.

• Reports any changes in the patient's condition to the Hospice Nurse and, when the patient resides in a facility, to the appropriate facility nurse. Documents the information as required, including the date and time of the report and the individual receiving the information.

• Assists or reminds patients to take medications in compliance with applicable regulations and the requirements of appropriate regulatory and accrediting bodies.

• Provides compassionate and respectful care to hospice patients and their families while maintaining patient dignity, comfort, and quality of life throughout end-of-life care.

• Communicates effectively with the Hospice Nurse and other members of the interdisciplinary care team regarding patient needs and changes in condition.

• Maintains patient confidentiality and complies with all applicable hospice policies, HIPAA requirements, and regulatory standards.

Education

• High School Diploma or equivalent.

Professional Experience & Requirements

• Must have a current state Certified Nursing Assistant (CNA) certification.

• Previous hospice or home health experience preferred.

• No prior hospice experience is required; one year of CNA experience is preferred.

• Must be willing and able to travel throughout the assigned territory.

• Must have a valid driver's license and reliable transportation.
