Job summary

Autumn Hospice in Weymouth, MA seeks a dedicated Certified Nursing Assistant (CNA) for the South Team. You will provide compassionate care, assist with ADLs, and support the interdisciplinary team to maintain patient comfort and dignity in end-of-life care.

Qualifications include a current CNA certificate, willingness to travel, and a valid driver’s license. Prior hospice experience is preferred, and a strong commitment to patient-centered care is essential.

Qualifications

  • Must have a current state CNA certification.
  • One year of CNA experience preferred.
  • Willingness to travel throughout the assigned territory.
  • Must have a valid driver's license and reliable transportation.

Responsibilities

  • Assists with ADLs including personal hygiene, grooming, mouth care, skin care, and nail care.
  • Assists with toileting, transfers, and positioning, including turning bed-bound patients.
  • Assists with prescribed exercises and ROM as instructed by Hospice Nurse.
  • Performs light housekeeping in the patient’s living area and helps with laundry.
  • Feeds and assists patients with meals and monitors comfort and safety.
  • Obtains vital signs as directed by the Hospice Nurse and reports changes.
  • Supports medication reminders in compliance with regulations and policy.
  • Provides compassionate, respectful care to patients and families and protects dignity.
  • Communicates patient needs and changes with the Hospice Nurse and care team.
  • Maintains patient confidentiality and follows HIPAA and hospice policies.

Now Hiring – Certified Nursing Assistant (CNA) – South Team

The Certified Nursing Assistant (CNA) will be responsible for, but not limited to:

Responsibilities
  • Provides or assists with activities of daily living (ADLs), including personal hygiene, grooming, mouth care, skin care, and nail care.

  • Provides assistance with toileting, positioning, and transfers, including frequent turning and positioning of bed-bound patients.

  • Assists with prescribed exercises and range-of-motion (ROM) activities that the patient and Hospice Aide have been instructed on by the Hospice Nurse.

  • Assists with light housekeeping of the patient's personal living area or room, including changing bed linens and patient laundry as needed.

  • Feeds and assists patients with meals as appropriate.

  • Obtains temperature, pulse, and respirations as directed and supervised by the Hospice Nurse.

  • Reports any changes in the patient's condition to the Hospice Nurse and, when the patient resides in a facility, to the appropriate facility nurse. Documents the information as required, including the date and time of the report and the individual receiving the information.

  • Assists or reminds patients to take medications in compliance with applicable regulations and the requirements of appropriate regulatory and accrediting bodies.

  • Provides compassionate and respectful care to hospice patients and their families while maintaining patient dignity, comfort, and quality of life throughout end-of-life care.

  • Communicates effectively with the Hospice Nurse and other members of the interdisciplinary care team regarding patient needs and changes in condition.

  • Maintains patient confidentiality and complies with all applicable hospice policies, HIPAA requirements, and regulatory standards.

Education
  • High School Diploma or equivalent.

Professional Experience & Requirements
  • Must have a current state Certified Nursing Assistant (CNA) certification.

  • Previous hospice or home health experience preferred.

  • No prior hospice experience is required; one year of CNA experience is preferred.

  • Must be willing and able to travel throughout the assigned territory.

  • Must have a valid driver's license and reliable transportation.
