About the Role:

• Assist residents with daily living activities such as bathing, dressing, grooming, and toileting

• Provide vital support with mobility and ambulation, including walking, transferring, and using assistive devices

• Monitor and report changes in residents' conditions to nursing staff and other healthcare professionals

• Maintain accurate records and documentation of resident care and services

• Collaborate with other healthcare team members to develop and implement individualized care plans

Responsibilities:

• Assist with vital signs, including blood pressure, pulse, and respiratory rate
• Provide emotional support and companionship to residents
• Aid residents with daily routines, such as feeding, toileting, and incontinence care
• Participate in team meetings and contribute to care plan development
