Duties and Responsibilities:

• Monitor residents' personal care needs and ensure they are met in accordance with their wishes
• Review and update care plans daily and communicate changes to relevant staff
• Report changes in residents' conditions to nurse supervisors
• Assist residents with daily needs and transportation to activities and appointments
• Participate in facility surveys and maintain confidentiality per HIPAA guidelines
