Walnut Creek Nursing and Rehab is seeking a compassionate Certified Nursing Assistant (CNA) to join our team for the 2nd shift. You will provide direct patient care, assist with daily living activities, and help monitor residents.

Qualifications

• Current certification as a Certified Nursing Assistant (CNA) in the state of Pennsylvania.
• High school diploma or equivalent required; some college or vocational training preferred.
• Ability to work effectively in a fast-paced environment with varying levels of patient acuity.
• Strong communication and teamwork skills.
• Ability to lift, transfer, and move patients as needed.

Responsibilities

• Provide direct patient care to residents, including assistance with daily living activities such as bathing, dressing, and grooming.
• Monitor and report changes in resident condition, including vital signs and other health metrics.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.

• Maintain accurate records and documentation of resident care and services.
• Promote a safe and healthy environment for residents and colleagues.
