Job summary

Signature HealthCARE at Summerfield Rehab & Wellness Center is seeking a Certified Nursing Assistant in Louisville, KY. Join a family-based organization delivering resident-centered care and a broad spectrum of services from short-term rehab to long-term care.

You will provide direct resident care under supervision, assist with ADLs, monitor vitals, and help maintain a compassionate environment. 8-hour shifts available, including 3 PM–11 PM, with strong team support and growth opportunities.

Qualifications

• Current, active Nursing Assistant certification from state where the facility operates.
• Effective verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under licensed staff.
• Customer service oriented with ability to work well under pressure.
• Approachable, flexible, and adaptable to change.
• Function independently, with integrity and stakeholder collaboration.

Responsibilities

• Perform direct resident care under supervision of licensed nursing personnel.
• Assist with activities of daily living (ADLs) per care plan.
• Change bed linen and maintain grooming supplies in resident rooms.
• Ambulate and transfer residents using appropriate devices and body mechanics.
• Record vital signs, weights, intake/output, and other basics as assigned.
• Turn and position residents regularly; support resident rights within care environment.
• Assist in orientation and training of other staff; handle special projects and duties as assigned.

How you Will make a difference

Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents in accordance with facility policies and procedures.

• Provide activities of daily living (ADLs) within the scope of practice as defined by state regulation and the resident’s plan of care.
• Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
• Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
• Routinely turn and position residents, as necessary.
• Recognize and support resident rights and choices within the care environment.
• Maintain established housekeeping standards within assigned duty areas.
• Promptly answer call lights and assist with resident needs.
• Assist in the orientation and training of other staff, as requested.
• Other special projects and duties, as assigned.

What you Need to make a Difference

• Current, active Nursing Assistant certification from state in which the facility operates.
