Qualifications

• Reliable attendance and ability to follow assigned schedules.
• Dress Code Scrubs are required while working in the facility.

Responsibilities

• Follow the resident assignment sheet and individual plan of care.
• Assist residents with activities of daily living, including meals, hygiene, and other personal care needs.
• Listen carefully to shift report and prioritize resident care appropriately.
• Communicate relevant resident care information to the licensed nurse throughout the shift.
• Notify the licensed nurse when leaving the unit and at the end of the shift.
• Communicate important information with coworkers to help maintain continuity and quality of resident care.
• Complete all required ADL documentation by the end of the scheduled shift.
• Follow facility policies, procedures, and resident care standards.
