Job summary

Heritage Park Care Center, part of Life Care Centers of America, is seeking a qualified Certified Nursing Assistant (CNA) in Carbondale, Colorado. Full-time, day and night shifts available, with a sign-on bonus of $3,000.

The facility is located a short drive from Glenwood Springs and Aspen, offering opportunities in short-term rehabilitation and long-term care. Responsibilities include providing routine nursing care, assisting with daily activities, and collaborating with a multidisciplinary

Qualifications

• High school diploma or equivalent.
• Graduate of a State-approved CNA program.
• Currently registered/licensed in applicable State.
• One year skilled nursing experience preferred.
• CPR certification upon hire or within orientation.

Responsibilities

• Provide routine daily nursing care to assigned patients.
• Assist with bathing, feeding, grooming, toileting, and mobility.
• Deliver direct care as directed while following Life Care standards.
• Maintain professional relationships with patients, families, and team.

As an important member of our interdisciplinary team, the CNA is responsible for providing routine daily nursing care to assigned patients in accordance with all applicable laws, regulations, and Life Care standards. Education, Experience, and Licensure Requirements

• High school diploma or equivalent
• Graduate of a State-approved CNA program
• Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
• One (1) year skilled nursing experience preferred
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
