Summary

Provides patient care support including hygiene, nutrition, ambulation, elimination, and safety as directed by the Charge Nurse, following established departmental guidelines. Documents appropriately into patient records based on skill level and standards. Completes admission procedures such as recording height, weight, vital signs, and orienting new residents. Ensures residents receive identification bands and admission kits. Responds promptly to call lights and resident needs. Performs rounds, repositions residents regularly, and takes vital signs and weights accurately, reporting findings to the Charge Nurse. Applies appropriate devices and performs treatments as instructed. Maintains supplies and equipment, and reports needs to the Charge Nurse or purchasing staff. Completes job duties by attending continuing education and in-services, accepting reassignments, and updating the Charge Nurse. Maintains effective working relationships and handles confidential information discreetly. Provides shift reports and participates in rounds. May be assigned additional duties as needed.

Job Qualifications

• High school diploma preferred, but not required.
• Active Certified Nurse Assistant (CNA) certification in the state of employment.
• CPR certification required or willing to obtain within six months of employment.
• Long-term care experience preferred.
• Excellent communication skills for interacting with patients, visitors, and staff.
• Ability to manage time and organize daily tasks to meet productivity standards.
• Physical ability to stand and walk for long periods.
• Ability to lift heavy objects with proper techniques and assistance as needed.
• Ability to transfer and move patients, including those weighing over 200 lbs, with assistance and proper techniques.
• Ability to push and pull equipment weighing over 40 lbs with assistance and proper techniques.
• Manual dexterity and coordination for working with patients and equipment.
• Ability to perform documentation accurately, either in writing or via tape recorder.
• Visual acuity to distinguish colors.

Pay range: $16.08 - $25.24 per hour
