Job summary

Sanford Health in Beaver Dam is seeking a Nursing Assistant (NA) to provide resident-centered care under the supervision of a charge nurse. The NA performs daily living assistance, takes vital signs, and supports mobility and dining in the resident’s care plan. You must hold a CNA certification, be at least 16 years old, and complete yearly competency testing.

Prior assisted living experience is preferred. The facility offers rotating shifts, 36 hours per week, and a comprehensive benefits

Qualifications

• High school graduate or GED preferred, but not required.
• Minimum age 16.
• Prior clinical or nursing assistant experience in assisted living preferred.

Responsibilities

• Assist residents with activities of daily living, including bathing, grooming, dressing, and mobility.
• Record and monitor vital signs and report changes to the charge nurse.
• Assist with meals, hydration, and documenting care and outcomes.
