White Oak Manor York in York, SC is seeking a Certified Nursing Assistant for full-time 1st shift (7a-3p) to support residents with daily living activities and medical needs. As part of a 109-bed skilled nursing and rehabilitation campus, you will follow care plans, monitor vital signs, assist with transfers, and document care while upholding infection control and compassionate, team-oriented care.

Responsibilities

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and feeding.
• Take and record vital signs (temperature, pulse, respiration, blood pressure).
• Observe and report changes in residents’ condition to the nurse.
• Provide mobility support and assist with transfers and ambulation.
• Follow care plans and physician/nursing instructions.
• Support infection control procedures, including hand hygiene and proper PPE use.
• Document resident care accurately and timely in clinical records.
• Assist with meals and hydration as needed.
