Job summary

Williamsport Health & Rehabilitation Center is seeking a Certified Nursing Assistant - I (CNA-I) to join our care team. The CNA-I will provide compassionate assistance with daily living activities and help maintain residents’ comfort and safety.

You will work with an interdisciplinary team to implement individualized care plans and ensure accurate documentation. The role emphasizes effective communication with residents, families, and providers, along with participation in quality improvement

Qualifications

• CNA-I certification required.
• High school diploma or equivalent required.
• Ability to work effectively in a fast-paced environment with minimal supervision.
• Excellent communication, interpersonal, and organizational skills.
• Ability to maintain confidentiality and handle sensitive information with discretion.

Responsibilities

• Provide excellent patient care to residents, including activities of daily living (bathing, dressing, grooming).
• Maintain accurate and up-to-date medical records and reports.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Communicate effectively with residents, families, and healthcare providers to ensure seamless care delivery.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
