Job summary

Care 360 Hospice is seeking a Hospice Certified Nursing Assistant to provide compassionate, patient-centered care in Southern Dayton and surrounding areas. The role includes travel to patients’ residences, bedside vigil support, and assistance with activities of daily living under the hospice aide plan of care.

The position offers Full-time hours, M-F, 7 am-3:30 pm, with salary $19-22/hr BOE and comprehensive benefits.

Qualifications

• Licensed Nursing Assistant in the state of practice (CNA)
• BLS Certification on record
• Experience in AL/LTC/SNF facility or Home Health preferred

Responsibilities

• Provide compassionate, patient-centered personal care (bathing, grooming, dressing, toileting, feeding) per the hospice aide plan of care
• Travel to patients' residences, including private homes and facilities, to deliver hospice services with compassion and professionalism
• Provide bedside vigil support during end-of-life care to ensure comfort and presence
• Assist with ambulation, transfers, positioning, and ROM exercises per the plan of care
• Safely use and assist with DME (walkers, wheelchairs, beds, lifts) and monitor changes in condition
• Document care accurately per Care 360 Hospice policies and report to the supervisor
• Participate in team meetings and educate families and caregivers within CNA scope
• Attend monthly educational meetings and ongoing training to maintain competency
• Other duties as assigned

Requirements of the Certified Nursing Assistant role:

• Licensed Nursing Assistant in the state of practice (CNA)
• BLS Certified
• Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
• Must be willing to TRAVEL
• Valid driver's license and Auto insurance Coverage with reliable transportation
• Hospice experience preferred

About the CNA role:

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
• Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
• Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.
• Accurately document care provided in accordance with Care 360 Hospice policies and procedures.
• Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
• Provide support and education to family members and caregivers within the scope of the CNA role.
• Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
