Job summary

Ascend Hospice & Palliative Care MA is seeking a Certified Home Health Aide (CHHA) to provide compassionate in-home care to our patients, assist with ADLs, toileting, hygiene, meals, and mobility, under Hospice Nurse supervision.

Candidates must hold a CHHA license in Massachusetts, be willing to travel, have a valid driver's license and transportation; prior hospice/home health experience preferred; we offer competitive salaries and comprehensive benefits.

Qualifications

• Must have State Certified CHHA license.
• Experience in hospice or home health preferred.
• No experience required but 1 year preferred.
• Willing to travel.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, positioning and transfers, turning and positioning of bed-bound residents/patients.
• Assists with prescribed exercise and ROM as taught by Hospice Nurse.
• Assists with light housekeeping of patient unit/room and laundry.
• Feeds and assists with meals; monitors intake.
• Obtains temperature, pulse and respirations under supervision.
• Reports changes to Hospice Nurse; documents on Home Health Aide Certification form.

Now Hiring - Certified Home Health Aide (CHHA)

This Certified Home Health Aide will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred

• Must be willing to travel
• Must have a valid driver's license and reliable transportation
