General Purpose
The primary purpose of this position is to provide assigned residents with routine daily nursing care and services according to their assessment and care plan, under the direction of supervisors.
Essential Duties
- Identify residents using wristbands or photo cards before administering treatments, serving meals, etc.
- Use only authorized abbreviations when recording information.
- Report any changes in residents' conditions to the Nurse Supervisor/Charge Nurse promptly.
- Record all entries on flow sheets, notes, charts, etc., clearly and descriptively.
- Report all accidents and incidents observed during the shift.
- Maintain confidentiality of residents' protected health information and report any violations.
- Report unauthorized attempts to access the facility's information system.
- Perform only those nursing procedures you are trained for.
- Prepare residents' rooms for receiving them, ensuring beds are made, tags are up, and admission kits are available.
- Greet residents and escort them to their rooms.
- Introduce residents to roommates and staff as appropriate.
- Make residents comfortable and assist with personal belongings, including packing and storing clothing.
- Transport residents within the facility and assist with loading/unloading from vehicles.
- Create a warm, positive, and calm environment throughout the unit.
- Participate in regular shift meetings to improve services and address issues.
- Address resident complaints and grievances.
- Participate in nursing reports and follow established policies on exposure to blood and body fluids.
- Make beds, add extra covers as requested, and record vital signs and measurements as instructed.
- Check frequently on residents unable to call for help and answer call bells promptly.
- Assist with feeding and record food and fluid intake, reporting any changes.
- Ensure water pitchers are clean, filled, and accessible.
- Perform after-meal care, including tray removal and personal hygiene.
- Follow work schedules and cooperate with inter-departmental staff to meet residents' needs.
- Notify the facility of lateness or absence, report exposures, and assist with residents' daily functions and mobility.
- Observe and report skin integrity issues and injuries, and assist with preparing residents for medical tests or examinations.
- Perform restorative procedures and assist with emergency evacuations if necessary.
- Use only trained equipment, report defective tools, and maintain confidentiality of all resident information.
- Report any resident abuse or property misappropriation, and honor treatment refusals with supervisor notification.
Supervisory Requirements
This position has no supervisory responsibilities.
Qualifications
- Minimum 10th-grade education.
- Ability to read, interpret, and communicate effectively in writing and verbally.
- Ability to apply basic mathematical concepts.
- Ability to solve practical problems and interpret instructions.
- Must be a licensed Certified Nursing Assistant according to state laws; CPR certification preferred.
Physical Demands
Must occasionally lift/move up to 25 pounds, and perform prolonged computer use. Regularly sit, stand, walk, and communicate. Assist in evacuations during emergencies.
Work Environment
Typically low to moderate noise levels; accommodations may be made for disabilities.
