We seek a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Home Health team. The CNA will provide essential personal care and support to patients in their homes, ensuring their comfort, safety, and overall well-being. This role requires strong interpersonal skills, empathy, and the ability to work independently while following care plans developed by the healthcare team.

Key Responsibilities

  • Personal Care Assistance:Provide direct personal care to patients, including bathing, grooming, dressing, toileting, and assistance with mobility. Ensure that patients' hygiene and comfort needs are met.
  • Vital Signs Monitoring:Measure and record patients’ vital signs, such as blood pressure, pulse, temperature, and respiration. Report any significant changes or concerns to the supervising nurse or healthcare provider.
  • Assistance with Activities of Daily Living (ADLs):Help patients with daily living activities, including eating, meal preparation, and light housekeeping tasks. Encourage and support patients in maintaining their independence.
  • Companionship and Emotional Support:Offer companionship and emotional support to patients, fostering a positive and comforting environment. Engage patients in conversation and activities to promote their mental well-being.
  • Communication:Maintain open and effective communication with patients, families, and the healthcare team. Report any changes in patients' conditions, behaviors, or needs to the supervising nurse.
  • Documentation:Accurately document care provided, patients’ conditions, and any changes in health status in the electronic health record (EHR) system. Ensure that documentation is completed in compliance with regulatory standards.
  • Mobility and Transfer Assistance:Assist patients with mobility and transfers safely, using appropriate techniques and equipment, such as wheelchairs, walkers, and transfer belts. Ensure patient safety during all movements.
  • Medication Reminders:Remind patients to take their medications as prescribed, ensuring they understand the importance of adherence to their medication regimen.
  • Compliance and Safety:Adhere to all safety protocols, infection control guidelines, and regulatory requirements while providing care in patients' homes. Maintain a clean and safe environment for patient care.
  • Collaboration with Healthcare Team:Work collaboratively with other members of the healthcare team, including nurses, therapists, and social workers, to provide comprehensive care tailored to each patient’s needs.
Qualifications

Qualifications

  • Education:High school diploma or equivalent required.
  • Certification:Valid Certified Nursing Assistant (CNA) certification in [State].
  • Experience:Previous experience in home health or a clinical setting preferred, but not required.
