Job summary

Your Health Organization is seeking a dedicated Certified Nursing Assistant (CNA) to support hospice patients across designated service areas, including home visits and ALFs.

You will ensure timely delivery of medications and DME, coordinate telehealth visits, and work closely with care teams to uphold care plans.

Qualifications

  • Certified Nursing Assistant certification in good standing.
  • Experience with phlebotomy preferred or willing to train.
  • Experience in home health or public health preferred.
  • Proficient with technology tools for remote monitoring, telemedicine, and EMR.

Responsibilities

  • Provide compassionate personal care to hospice patients in homes including bathing, grooming, oral hygiene, dressing and toileting.
  • Assist with mobility, transfers, and fall prevention; perform light housekeeping and linen care.
  • Monitor patient conditions, document changes, and report to hospice nurse promptly.
  • Support ADLs and provide emotional support to patients and families; assist with meals and hydration.
  • Deliver medications and DME, review instructions, and coordinate telehealth visits with providers.

Job description

We are looking for a dedicated candidate join our team as a Certified Nursing Assistant CNA to service our hospice patients in the LOCATION area The Certified Nursing Assistant CNA serves as a vital link between patients healthcare services and community resources ensuring seamless access to care and improving the quality of service delivery This role includes visiting patients in their homes and Assisted Living Facilities ALFs to facilitate visits with providers and specialty services Additionally the CNA is responsible for delivering medications and durable medical equipment DME to ensure patients have timely and safe access to essential supplies CNA perform visits in homes and facilities ALF and ILFs in their designated service area You must have reliable transportation as travel is required daily This is a full time salary based working 8 hour shifts 800AM 500PM; Monday Friday The following service areas are available

What Are We Looking For

Responsibilities
  • Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care planCommunicate effectively via phone email Microsoft Teams Athena in person etc
  • Provide compassionate personal care to hospice patients in their homes including bathing grooming oral hygiene dressing and toiletingAssist patients with safe mobility transferring repositioning and fall prevention supportPerform light housekeeping tasks related to patient care changing linens cleaning care areas laundering patient items
  • Monitor and document patient condition changes including skin integrity appetite comfort levels and safety concernsReport any significant changes in patient condition to the hospice nurse or clinical team promptly
  • Support patients with activities of daily living ADLs while promoting dignity comfort and independenceProvide emotional support and companionship to patients and their familiesAssist with meal preparation feeding and ensuring adequate hydration based on care plans
  • Deliver medications to patients homes review instructions and perform medication reconciliation ensuring accuracy between current medication lists and what is found in the homeCoordinate and facilitate telehealth visits between patients and pharmacists specialists therapists and primary care providers ensuring patients receive timely carePerform routine venipuncture for laboratory testing and ensure proper specimen handling and labelingCollect blood specimens during home or facility visits as needed to support patient care and diagnostics
Qualifications
  • Certified Nursing Aide required
  • Certifications or licenses must be in good standing
  • Phlebotomy experience preferred
  • If not experienced in phlebotomy candidate must be willing to complete training and demonstrate competency in the skill
  • Previous experience in public health or home health preferred
  • Experience with DME delivery or logistics operations preferred
  • Must be proficient in using technology tools and platforms for remote patient monitoring telemedicine and electronic medical records
  • Strong organizational and time management skills with the ability to multitask and prioritize responsibilities effectively
  • Ability to read and communicate effectively
  • Strong written and verbal communication skills
  • Basic computer knowledge
  • Must be able to work independently with minimal supervision and direction
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances
  • Must be able to work flexible hours and travel between offices facilities etc
  • Must be a licensed driver with an automobile that is insured in accordance with state andor organization requirements and is in good working condition
  • Must have a clean driving record and experience operating vehicles in a professional capacity
