Job summary

Your Health Organization is seeking a dedicated Continuous CNA to support hospice patients in the Savannah region. You will work closely with the interdisciplinary team to stabilize symptoms, provide comfort, and ensure dignity and safety in patients' homes.

The role involves travel between patient homes, 7 on 7 off 12-hour shifts, and a full-time, salary-based arrangement. Eligible candidates will have CNA credentials, with phlebotomy and home health experience preferred.

Qualifications

  • Certified Nursing Assistant certification in good standing.
  • Phlebotomy experience preferred; willingness to train if not experienced.
  • Public health or home health experience preferred.
  • Experience with DME delivery or logistics operations preferred.
  • Proficient with technology tools for remote patient monitoring, telemedicine, and EMRs.
  • Strong organizational and time management skills; multitasking capability.
  • Excellent reading and written communication skills.
  • Basic computer knowledge.
  • Ability to work independently with minimal supervision.
  • Good interpersonal and communication skills under all conditions.
  • Must be able to travel between offices/facilities and have a licensed, insured vehicle.

Responsibilities

  • Collaborate with the hospice care team to ensure coordinated patient care.
  • Communicate effectively via phone, email, Teams, and in person.
  • Provide compassionate personal care to hospice patients in their homes.
  • Assist with safe mobility, transfers, and fall prevention.
  • Perform light housekeeping related to patient care.
  • Monitor and document patient condition changes and safety concerns.
  • Report significant changes to the hospice nurse promptly.
  • Support ADLs with dignity and independence.
  • Provide emotional support to patients and families.
  • Assist with meal preparation, hydration, and feeding per plan.
  • Deliver medications, review instructions, and reconcile lists.
  • Coordinate telehealth visits with pharmacists, specialists, and providers.
  • Perform routine venipuncture for labs; handle specimens correctly.
  • Collect blood samples during home or facility visits.

We are looking for a dedicated candidate join our team as a Continuous (CNA) to service our hospice patients in the Savannah area. What Are We Looking For?

Areas of Responsibility:
  • Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care plan.
  • Communicate effectively via phone, email, Microsoft Teams, Athena, in-person, etc.
  • Provide compassionate personal care to hospice patients in their homes, including bathing, grooming, oral hygiene, dressing, and toileting.
  • Assist patients with safe mobility, transferring, repositioning, and fall-prevention support.
  • Perform light housekeeping tasks related to patient care (changing linens, cleaning care areas, laundering patient items).
  • Monitor and document patient condition changes, including skin integrity, appetite, comfort levels, and safety concerns.
  • Report any significant changes in patient condition to the hospice nurse or clinical team promptly.
  • Support patients with activities of daily living (ADLs) while promoting dignity, comfort, and independence.
  • Provide emotional support and companionship to patients and their families.
  • Assist with meal preparation, feeding, and ensuring adequate hydration based on care plans.
  • Deliver medications to patients’ homes, review instructions, and perform medication reconciliation, ensuring accuracy between current medication lists and what is found in the home.
  • Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers, ensuring patients receive timely care.
  • Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.
  • Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.
Qualifications:
  • Certified Nursing Aide required. Certifications or licenses must be in good standing.
  • Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.
  • Previous experience in public health or home health preferred.
  • Experience with DME delivery or logistics operations preferred.
  • Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively. Strong written and verbal communication skills.
  • Basic computer knowledge.
  • Must be able to work independently with minimal supervision and direction.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.
  • Must have a clean driving record and experience operating vehicles in a professional capacity
