Job summary

Heritage Woods of Plainfield is seeking a Full Time CNA in Plainfield, IL. The role involves delivering personalized care per each resident's plan, assisting with ADLs, and reporting changes to the clinical team.

You will engage with residents and families compassionately, participate in ongoing education, and uphold confidentiality. A CNA certification and a High School diploma or GED are required.

Qualifications

• Current CNA certification from an accredited program.
• Strong communication and interpersonal skills.
• Ability to provide care with love, compassion and dignity.

Responsibilities

• Deliver individualized resident care per care plans.

Responsibilities:

• Deliver exceptional and individualized care as determined by each resident's care plan
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity
• Report changes in resident health status to the clinical team
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
• Comply with all rules and regulations regarding confidentiality and privacy of resident information
• Perform any other duties as assigned

QualificationsQualifications:
