Location: Des Moines, IA 50314

Shift: CNA FT 6AM - 6PM, including every other weekend

Position Purpose

The CNA is responsible for providing residents with the highest quality of direct routine care in all activities of daily living, under the supervision of the LPN or RN and in accordance with the Nursing Assistant Program standards, the Resident’s Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program.

Essential Functions of Position
Quality of Care
  • Provide ongoing resident care upholding and promoting the resident’s right to be free from abuse, neglect, and misappropriation of resident property.
  • Build quality relationships and communicate competently on a one-to-one basis while integrating the resident-centered Plan of Care.
  • Demonstrate sensitivity to physical, mental, and psychosocial needs while maintaining dignity, performing directed nursing, personal care, and restorative services.
  • Support residents’ rights and functional independence.
  • Monitor and report resident care outcomes, ensuring accurate documentation.
  • Provide the highest quality care to all residents, regardless of race, gender, sexual orientation, religion, age or cultural differences.
  • Maintain awareness of facility QAPI goals and promote the QAPI program.
  • Practice a strong culture of customer service, ensuring a resident-centered experience.
Basic Nursing, Personal Cares and Basic Restorative Services
  • Facilitate the Plan of Care through completion of nursing responsibilities and communication of resident needs to LPN/RN.
  • Complete basic nursing cares: vital signs, height/weight, reporting abnormal changes, caring for residents who are actively dying.
  • Perform personal care: bathing, grooming, mouth care, dressing, toileting, eating, hydration, skin care, transfers, positioning and turning.
  • Assist with nutritional intake: feeding, monitoring intake/output, serving supplemental nourishment, upholding dietary restrictions.
  • Perform basic restorative services: training residents in self-care, use of assistive devices, range of motion, turning, training for bowel and bladder, care of prosthetic/orthotic devices.
  • Assist nurses with special procedures and care conferences.
  • Ensure nursing care activities follow Nursing Assistant Program standards while protecting resident dignity, privacy, and autonomy.
  • Facilitate assistance in activities of daily living and conflict resolution.
  • Engage in meaningful conversation with residents based on their preferences.
  • Maintain security of residents’ personal possessions.
  • Interact politely with residents’ families, visitors, and interdisciplinary care team.
  • Recognize limits of knowledge and facilitate situations beyond CNA competency.
Compliance – Safety, Documentation, HIPAA
  • Promptly respond to call lights.
  • Use proper body mechanics and encourage correct practices among staff.
  • Adhere to safety policies for lifting, positioning and equipment use, including transfer belts, Hoyer or mechanical lifts.
  • Strictly follow Standard Precaution infection control procedures and OSHA bloodborne pathogen guidelines.
  • Maintain a clean environment in resident rooms, following infection control procedures.
  • Avoid restraints in accordance with DHS nursing home standards.
  • Document all cares accurately and securely; adhere to HIPAA privacy regulations.
  • Report and document incidents and errors to relevant staff.
  • Prioritize work assignments based on staff levels and resident acuity.
  • Review and complete shift reports and delegate tasks appropriately.
