Job summary

Altea Healthcare is seeking a Certified Nurse Practitioner (NP) for post-acute and skilled nursing care across facilities in the South Bend, IN area. The role offers a base salary of $115,000–$140,000 with uncapped bonus potential and full-time hours.

You will diagnose, treat, and guide patients, collaborating with a multidisciplinary team to ensure high-quality care. Travel is required to multiple facilities in the South Bend region.

Qualifications

• NP license (or eligibility to obtain)
• Current DEA registration or willingness to obtain prior to start date
• All experience levels welcome – Training & mentorship available
• Passion for geriatrics, internal medicine, or primary care
• Strong team player with excellent communication skills
• Self-motivated with a patient-first approach

Responsibilities

• Perform comprehensive patient assessments and physical exams in post-acute care settings.
• Diagnose and manage acute and chronic conditions in skilled nursing facilities.
• Order, interpret, and act on diagnostic tests (labs, imaging).
• Prescribe medications and develop individualized treatment plans.
• Collaborate with physicians, nurses, and facility staff to coordinate care.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
