Touchstone Health Hospice in San Antonio seeks a Certified Nurse Assistant to provide hands-on, compassionate care to patients in their homes. You will assist with activities of daily living, monitor vitals, and support dignity and comfort during a meaningful stage of life.

The role emphasizes building real relationships with patients and families, delivering consistent, patient-centered care, and collaborating with licensed nurses.

Qualifications

• Current Texas CNA certification is required.
• Completion of a state-approved Nurse Aide Training program.
• High school diploma or equivalent.

Responsibilities

• Provide personal care and activities of daily living under supervision of a licensed nurse.
• Assist with bathing, grooming, dressing, mobility, toileting, and feeding.
• Measure and record vital signs: temp, pulse, resp, BP.
• Observe changes and report to supervising staff promptly.
• Assist with ambulation and transfers to ensure comfort.
• Maintain accurate care documentation in the EMR.
• Offer emotional support and companionship to patients and families.
• Participate in in-service training and continuing education.

About the Role

As a Certified Nurse Assistant with Touchstone Health's Hospice team in San Antonio, you'll provide hands-on, compassionate care to hospice patients in the comfort of their own homes. You'll support activities of daily living, monitor vital signs, and offer the kind of steady, personal presence that helps patients maintain dignity and comfort at one of life's most meaningful stages. This role is a natural fit for someone who wants to build real relationships with the patients and families they care for, not just check tasks off a list.

Key Responsibilities

• Provide personal care services in accordance with each patient's plan of care and under the supervision of a licensed nurse
• Assist patients with bathing, grooming, dressing, mobility, toileting, and feeding
• Measure and record vital signs including temperature, pulse, respiration, and blood pressure
• Observe and report changes in patient condition or care needs promptly to supervising staff
• Assist with ambulation, transfers, and positioning to promote comfort and prevent complications
• Maintain accurate documentation of care provided in the EMR
• Provide emotional support and companionship to patients and families
• Participate in in-service training and continuing education

Requirements: What We're Looking For

• Current certification as a Nurse Aide in the state of Texas
• Completion of a state-approved Nurse Aide Training program
• High school diploma or equivalent
• At least one year of experience preferred; hospice experience a plus
• Strong communication and interpersonal skills with patients, families, and staff
• Reliable transportation, valid driver's license, and auto insurance for home visits
