Social Service Associate/Discharge Planner at Solaris Charlotte Harbor needed

Preferably a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology.

Will preferably have one (1) year supervised social work experience in a health care setting working directly with individuals, discharge planning preferred.

Some of your Certified Nursing Assistant (CNA) Job Responsibilities include:

• Assisting residents with ADLs, and work with the team to ensure appropriate resident care is provided
• Assisting at meals
• Following infection control and center policies
• Measuring and recording heights and weights temperatures, pulses, respirations and blood pressures (TPRs and B/P) as instructed
• Reports all changes in residents’ condition to supervisor as soon as practical
• CNA (Certified Nursing Assistant) Certification by the State
