Qualifications

Responsibilities

• Collaborate with interdisciplinary team and communicate with patients, families and staff.
• Assess patient condition and document findings.
• Use professional judgment to treat patients and contact physician as needed.
• Review health risks and nutrition issues with patients; discuss lifestyle changes.
• Schedule consultations, referrals and prescriptions as directed.
• Educate patient and family on care plan and ensure understanding.
