Endeavor Health is seeking a Certified Medical Assistant in Warrenville, IL. The role supports patients with care management, education, and coordination of services under supervision.

Qualifications

  • 2 years of experience in outpatient ambulatory care setting.
  • 1 year of Epic user experience.
  • Knowledge of insurance/payor mix.
  • Experience with healthcare systems and community-based organizations.
  • Strong organizational and communication skills.

Hourly Pay Range

$22.14 - $33.21 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights
  • Position: Certified Medical Assistant
  • Location: Warrenville, IL
  • Full Time: 40 hours per week
  • Hours: Monday-Friday, 8-hour shifts, 8:00a-4:30p
A Brief Overview

Under the general supervision this position works independently and as part of a team to provide support, health education, and care management services to patients and their families using an empowerment approach in coordination with health care providers and systems. Responsible for wellness coaching to reduce or eliminate high risk behaviors; encourages the adoption of habits that are conducive to a higher quality of life; develops and distributes health education materials to identified patients and family members; discusses customized care plans for patients including goals and plans of action; coordinates care with other health related vendors for optimal patient care.

What You Will Do
  • Provides support, empowerment, education, and care management services to identified patients as designated.
  • Verifies eligibility based on criteria report
  • Identifies support needs for patients and/or their caregivers/families.
  • Develops and/or modifies care plans, provides creative guidance in initiating care plan, assists patient in overcoming any self-management challenges based on provider’s plan of care.
  • Conducts assessment of patients' physical activity, dietary habits, cigarette smoking habits, and social and economic resources.
  • Works with patients with readmission issues to prevent reoccurrence.
  • Provides prescriptions, DME equipment and outpatient services to appropriate patients as needed.
  • Assists in reviewing all specifically designated patients such as readmissions, CHF patients, AMI patients, PNA patients, COPD patients and other high risks designated individuals as assigned
  • Documents all actions in required EMR system.
  • Maintains daily case load and discusses any complex patients with RN Care Manager that may need higher level of care management.
  • Coordinates with RN Care Manager - develops and modifies care plans, provides creative guidance in initiating care plan, overcoming any self-management challenges as needed.
  • Utilizes and works with community resources to ensure resources available to the patient and their disease specific care options
  • Requests referrals from provider for needed services and facilitates access to community resources.
  • Collaborates with provider and RN Care Manager to help educate patients with chronic illness about evidence-based standards of care and self-management of their chronic illness.
  • Participates in performance improvement activities as assigned.
  • Complies with all required documentation of patient services.
  • Provides baseline and benchmarks for Quality improvement/outcome tracking.
What You Will Need
  • 2 Years of experience in outpatient ambulatory care setting
  • 1 Year of epic User experience
  • Knowledge and understanding of insurance/payor mix
  • Experience working with healthcare systems and community-based organizations.
  • Ability to handle multiple responsibilities, stress, and high work load volumes
  • Ability to easily adapt to change
  • Working knowledge of Microsoft Office products
  • Ability to work as part of a multidisciplinary team
  • Strong organizational skills with excellent written and oral communication skills
  • Ability to work effectively with staff, community members, family members, and external agencies
  • Demonstrated problem solving and decision-making skills with ability to exercise initiative
  • Knowledge and understanding of continuous quality improvement principles
  • Current State of Illinois Licensed Practical Nurse licensure Required And
  • CPR or BLS - American Heart Association (AHA) Preferred
  • Current CMA certification or registration Required Or
