Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our team.

We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities, and private homes. Our team treats residents, families, and loved ones with kindness, respect, compassion, and dignity. We offer comprehensive support to residents and staff.

Job Responsibilities

• Assist with prescribed exercises and ROM activities as taught by the Hospice Nurse.
• Provide assistance with toileting, positioning, transfer activities, and turning/positioning of bed-bound residents/patients.
• Assist with ADL activities including personal hygiene, grooming, mouth, skin care, and nails.
• Remind or assist patients with medication intake in compliance with regulations.
• Help with light housekeeping of the patient's personal unit or room, including changing bed linens and laundry.
• Feed and assist patients with meals.
• Obtain temperature, pulse, and respiration readings under supervision.
• Report any changes in the patient's condition to the Hospice Nurse and document the information accurately.

Ideal Candidates Will Have:

• HHA License in the state of NJ
• Understanding of hospice philosophy
• Minimum of one year hospice experience
• Dependable transportation
