Certified Home Health Aide - CHHA - CNA Hospice - Middlesex

Join our team as a Certified Home Health Aide (CHHA) at Preferred Care Health Centers. We are seeking dedicated professionals to provide compassionate hospice care in Middlesex County.

Position Details

About the Role

Swan Hospice is looking for CHHA with hospice experience to join our team. We deliver quality hospice care in skilled nursing facilities, assisted living facilities, and private homes, emphasizing kindness, respect, compassion, and dignity. Our comprehensive team provides support and stability to residents and staff.

Responsibilities
  1. Assist with prescribed exercises and ROM activities as instructed by the Hospice Nurse.
  2. Provide assistance with toileting, positioning, transfer activities, and turning of bed-bound residents/patients.
  3. Assist with ADL activities, including personal hygiene, grooming, mouth, skin care, and nails.
  4. Remind or assist patients with medication intake in compliance with regulations.
  5. Perform light housekeeping, including changing bed linens and laundry.
  6. Feed and assist with meals.
  7. Record temperature, pulse, and respiration under supervision.
  8. Report any changes in the patient’s condition to the Hospice Nurse and document accordingly.
Ideal Candidates
  • HHA License in NJ
  • Understanding of hospice philosophy
  • Minimum one year hospice experience
  • Dependable transportation
