Job summary

Synergy HomeCare of Teaneck, NJ seeks a compassionate Certified Home Health Aide (HHA) to provide one-on-one care in patients' homes, supporting daily activities and wellbeing. You will build relationships, monitor care, and report observations to supervisors in a timely manner.

Responsibilities include assisting with mobility, personal care, housekeeping, documentation, and engaging patients in exercises to promote mental alertness. CPR preferred and a valid HHA program degree is required.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED.
• One year of professional experience.
• Driver’s license required.
• CPR certification preferred.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medication reminders
• Engage patients in exercises or other activities

Education
