• Home Health Aide certification in the State of New Jersey required.
• BLS required within 30 days from date of hire or transfer.
• Valid NJ driver's license, valid registration and insurance required.
• Safe driving record also required.
• Minimum 1 year working with the frail or elderly.
• Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and skill will be required within 30-60 days from date of hire.

Job Summary

The PACE LIFE Program Home Health Aide provides and assists participants with activities of daily living, restorative and supportive care necessary to preserve the home environment. The position reports the health status and provides a safe environment. The Home Health Aide ensures patient care is in accordance with the plan of treatment assigned by the primary nurse, and complies with other established standards.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

Responsibilities

Assist participants with activities of daily living and deliver restorative and supportive care in accordance with the plan of treatment. Report patients' health status, maintain a safe environment, and collaborate effectively with the interdisciplinary team.

Performance Expectations

• Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.

Work Environment

This position must work effectively within an interdisciplinary team model, interfacing with a wide range of clinical and social services disciplines. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Requirements: corrected vision and hearing to normal range; ability to communicate with all staff, vendors, and clients; manual and finger dexterity and eye-hand coordination; ability to use department equipment; lifting/carrying up to 30 pounds. Use appropriate body mechanics and equipment with accommodation if needed.

This position reports to department leadership.
