Job summary

Live Long Well Care is seeking Certified Home Health Aides (CHHA) for part-time and full-time positions to provide personal care and companionship to seniors.

Both internal and external applicants are welcome; weekend shifts are available. A valid driver’s license and clean driving record are required, and staff must have an up-to-date smartphone for timekeeping and care documentation.

Qualifications

• High school graduation or GED required.
• Must have completed a Certified Home Health Aide program.
• Two years’ experience in a similar position desired.
• Ability to read, write, speak, and understand English fluently.
• Must have an updated smartphone for time/attendance and care documentation.
• Valid driver’s license with clean driving record.

Responsibilities

• Provide general assistance and care to patients/clients per the non-personal care plan.
• Plan meals, shop for food and household supplies, and prepare nutritious meals.
• Maintain a clean, safe, and orderly home environment for the patient/client and family.
• Provide companionship and medication reminders.
• Transport patients/clients when directed by supervisor.
• Accurately document services and report changes to supervisor.
