Job summary

Boost Home Healthcare is seeking a Certified Home Health Aide (CHHA) to join our compassionate team. You will provide personal care in patients' homes, document visits, and report changes to the RN to support safe, in-home recovery.

Qualifications

• Requires Certified Home Health Aide credentials in the state of operation.
• Must have current BLS Certification.
• One year of healthcare experience in settings such as home health, hospice, or long-term care.
• Access to reliable transportation is required.

Responsibilities

• Provide personal care and hands-on assistance in the patient’s home.
• Complete visit records in a timely manner per agency policy.
• Report changes in patient condition to the RN.
• Perform household tasks essential to health care in the home as assigned.
• Assist with activities and medications as appropriate.
• Perform simple procedures as an extension of therapy or nursing services as assigned.

The Certified Home Health Aide (CHHA) is a paraprofessional person who is specifically trained, competent, and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the Registered Nurse (RN).

What you’ll be doing:

• Perform personal care, bath and hands-on care as assigned.
• Complete appropriate visit records in a timely manner as per Agency policy.
• Report changes in the patient’s condition and needs to the RN.
• Perform household services essential to health care in the home as assigned.
• Ambulate and exercise the patient as assigned and assist with medications that are ordinarily self-administered as assigned.
• Perform simple procedures as an extension of the therapy or nursing services; e.g. range of motion (ROM) exercises as assigned.
