Job summary

Boost Home Healthcare - Joliet is seeking a Certified Home Health Aide (CHHA) to provide compassionate personal care in patients’ homes. You will perform basic nursing tasks, assist with daily activities, and coordinate with RNs to ensure quality in-home care.

Qualifications

• Must have Certified Home Health Aide credentials in the state of operation.
• Current BLS Certification.
• One Year of Healthcare experience in settings such as Home Health, Hospice, or Long Term Care.
• Access to reliable transportation required.

Responsibilities

• Perform personal care and basic nursing tasks as assigned.
• Complete visit records in a timely manner as per Agency policy.
• Report changes in the patient’s condition and needs to the RN.
• Perform household services essential to health care in the home as assigned.
• Ambulate and exercise the patient as assigned and assist with medications that are ordinarily self-administered as assigned.
• Perform simple procedures as an extension of the therapy or nursing services; e.g. ROM exercises as assigned.
