Role Description

The Certified Home Health Aide role is a full-time, on-site position based in Freehold, NJ, providing direct care to clients in their homes within the service area. Day-to-day responsibilities include assisting clients with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support. The role involves delivering personal care, monitoring and reporting changes in client condition, and supporting basic patient care under the supervision of nursing staff. The Certified Home Health Aide helps with light housekeeping, meal preparation, and companionship to promote safety, comfort, and emotional well-being. This position also requires clear documentation of services provided and effective communication with clients, families, and the care team.

Qualifications

• Candidates should possess strong Home Care and Patient Care skills, including experience working in clients’ homes or community-based settings.
• Candidates should possess skills in Activities of Daily Living, such as assisting with bathing, dressing, toileting, mobility, and safe transfers.
• Candidates should possess Personal Care skills, including hygiene support, grooming, and maintaining client comfort and dignity.
• Candidates should possess Communication skills, including clear verbal and written communication, active listening, and the ability to follow care plans and instructions.
• Current certification as a Home Health Aide (CHHA) in New Jersey; CNA certification is a plus.
• Ability to work collaboratively with nurses, physicians, clients, and families while maintaining professional boundaries.
• Reliable transportation to travel to client homes within the service area and ability to work assigned shifts, which may include evenings or weekends.
• Basic understanding of infection control, safety practices, and confidentiality requirements (HIPAA).
