Job summary

SYNERGY HomeCare is seeking a compassionate Caregiver to support clients at home, helping with daily activities, mobility, and independence in a one-on-one setting.

You will follow individualized care plans, provide companionship, monitor wellbeing, and assist with transportation and documentation, while enjoying paid training and growth opportunities in a supportive environment.

Qualifications

• High School Diploma or GED required.
• At least 18 years old.
• CPR and First Aid certification required.
• HHA license preferred.

Responsibilities

• Assist with daily living activities including hygiene, meals, and mobility.
• Follow ICPs tailored to each client's needs.
• Provide companionship, emotional support, and social engagement.
• Assist with medication reminders and monitor wellbeing.
• Support transportation to appointments and community outings.
• Maintain accurate care documentation and follow guidelines.
