Job summary

SYNERGY HomeCare in New Jersey is seeking a dedicated Caregiver to support clients in their homes with daily activities, personal care, and mobility. You will follow Individualized Care Plans and provide compassionate, one-on-one support to help clients maintain independence.

Ideal candidates are 18+, with a HS diploma or GED, CPR/First Aid, a valid driver’s license, and reliable transportation. Prior home health experience is preferred, and opportunities for training and career advancement are

Qualifications

• High School Diploma or GED.
• At least 18 years of age.
• Prior home health aide experience preferred.
• Valid driver’s license and reliable transportation.
• CPR and First Aid certification.
• HHA license preferred.

Responsibilities

• Assist with daily living activities such as personal hygiene, meal preparation, and mobility.
• Follow ICPs tailored to each client's needs.
• Provide companionship, emotional support, and social engagement.
• Assist with medication reminders and monitor overall well-being.
• Support clients with transportation to appointments and community outings.
• Maintain accurate care documentation and follow care guidelines.
