We are seeking a dedicated and empathetic Certified Diabetes Educator to join our healthcare team. In this role, you will empower patients diagnosed with pre-diabetes, Type 1, and Type 2 diabetes to take control of their health through comprehensive self-management education, lifestyle modification, and clinical guidance. This position does require CDCES to travel to various clinics throughout the APHS System. Reliable transportation is required. Top of Form
· Licensure / Certification:
o Current, active license as a Registered Nurse (RN), Registered Dietitian Nutritionist (RDN), Pharmacist, Exercise Physiologist or other allied health professional.
o Certified Diabetes Care and Education Specialist (CDCES) credential preferred (or eligibility to sit for the exam within a specified timeframe).
· Experience: Preferred minimum of 1–2 years of clinical experience in diabetes education, endocrinology, or outpatient care.
· Skills:
o Strong proficiency with modern diabetes technology (CGMs, insulin pumps, smart pens).
o Excellent interpersonal, verbal, and written communication skills with a compassionate approach to patient care.
o Ability to translate complex medical terminology into clear, actionable advice for patients of diverse backgrounds.
o Preferred bilingual in Spanish and English, but not required
The physical demands described here represent those that must be met by an employee to successfully perform the essential functions of this job.
1. Maintain accurate documentation in the patient’s EHR based on established documentation procedures.
2. Fulfill clerical responsibilities as assigned including but not limited to the following: obtaining lab/X-ray reports, hospital notes, and referral information. Complete forms/requisitions as needed; verify patient demographics.
3. Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.
4. Assist Medical Providers with communicating with patients regarding diabetic medications, diabetic symptoms, referrals, appointments, and paperwork.
5. Ensure culturally and linguistically appropriate services.
6. Represent the company professionally and positively to enhance and promote the core values and mission of the organization.
7. Participate in mandatory in-services and drills, attend staff meetings and other trainings, including appropriate age specific educational trainings offered on regular basis for professional development.
8. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
