Summary of Position
We are seeking a dedicated and empathetic Certified Diabetes Educator to join our healthcare team. In this role, you will empower patients diagnosed with pre-diabetes, Type 1, and Type 2 diabetes to take control of their health through comprehensive self-management education, lifestyle modification, and clinical guidance. This position does require CDCES to travel to various clinics throughout the APHS System. Reliable transportation is required.
Minimum Qualifications
- Licensure / Certification:
- Current, active license as a Registered Nurse (RN), Registered Dietitian Nutritionist (RDN), Pharmacist, Exercise Physiologist or other allied health professional.
- Certified Diabetes Care and Education Specialist (CDCES) credential preferred (or eligibility to sit for the exam within a specified timeframe).
- Experience:
- Preferred minimum of 1–2 years of clinical experience in diabetes education, endocrinology, or outpatient care.
- Skills:
- Strong proficiency with modern diabetes technology (CGMs, insulin pumps, smart pens).
- Excellent interpersonal, verbal, and written communication skills with a compassionate approach to patient care.
- Ability to translate complex medical terminology into clear, actionable advice for patients of diverse backgrounds.
- Preferred bilingual in Spanish and English, but not required
Physical Requirements
- Continuous walking, standing and moving about.
- Frequently bends, kneels and crouches.
- Repetitive movement of hands and fingers – typing and/or writing.
- Ability to communicate effectively.
- Possible exposure to hazardous fumes, airborne particles, toxic/caustic chemicals, bodily fluids, etc.
Key Responsibilities
- Maintain accurate documentation in the patient’s EHR based on established documentation procedures.
- Fulfill clerical responsibilities as assigned including but not limited to the following: obtaining lab/X-ray reports, hospital notes, and referral information. Complete forms/requisitions as needed; verify patient demographics.
- Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.
- Assist Medical Providers with communicating with patients regarding diabetic medications, diabetic symptoms, referrals, appointments, and paperwork.
- Ensure culturally and linguistically appropriate services.
- Represent the company professionally and positively to enhance and promote the core values and mission of the organization.
- Participate in mandatory in-services and drills, attend staff meetings and other trainings, including appropriate age specific educational trainings offered on regular basis for professional development.
- Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
- Maintain safe, secure, and healthy work environment by following and enforcing established policies and procedures and the ability to professionally encourage others to do the same to support a healthy team atmosphere.
