Job summary

Blehealth,-LLC is seeking a full-time Community Health Worker for our Pomona office. The role is 40 hours per week, Monday through Friday, 8am-5pm, with travel to clients' homes and community sites. Must be certified CHW, possess a valid driver's license and reliable transportation.

CPR certification, ability to work a flexible schedule, and successful Live Scan/background check are required. Benefits post-probation include medical/dental/vision and life insurance; 401k eligibility after six

Qualifications

• CHW certification required.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• TB test negative.
• Live Scan fingerprint and background check.
• Flexible schedule as needed.

Responsibilities

• Assist clients in homes, community, or clinic settings.
• Travel extensively to client homes and outreach sites.
• Explain program purposes and potential impact on well-being.
• Identify socio-economic factors affecting health and plan goals.
• Educate on ER use and alternatives.
• Coach chronic condition self-management and care plan understanding.
• Help access health-related services and medical homes.
• Document encounters and submit monthly reports.
• Maintain electronic client files and track service plans.
