External Job Description

• Provides various personal caregiving services in accordance with an established plan of care.
• Ensures the personal needs and comfort of clients in their own homes.
• Assists clients with activities of daily living such as bathing, dressing, grooming, and toileting.
• Prepares meals and manages special diets as required.
• Performs general housekeeping activities.
• Administers medications to clients, following training and delegation by a Nurse, with proper certification and licensing (e.g., Nurse Delegation Core and/or Nurse Delegation Diabetes course).
• Provides Passive Range of Motion exercises, requiring current credentials, proof of FCG or BT, and annual CE completion.
• Assists with taking vital signs for informational purposes; does not record readings.
• Provides transportation assistance to and from activities.
• Offers compassionate companionship to clients.
• Performs other duties as assigned.

Qualifications

• HS/GED preferred.
• Caregiver License required (HCA/CNA), renewed annually.
• Ability to comply with background requirements.
• Excellent verbal and written communication skills.
• Ability to handle confidential information responsibly.
