Qualifications

  • High school graduate or equivalent.
  • Verifiable, certified caregiver within the State of Arizona.
  • Current fingerprint clearance card through the Arizona Department of Public Safety.
  • Current CPR and First Aid certification.
  • Memory care training, as required by the Department of Health Services (or able to receive within 30 days of hire).
  • Previous experience in an assisted living setting is preferred.

Responsibilities

  • Follows the individualized service plan and assists with ADLs such as dressing, feeding, and toileting.
  • Helps ambulate residents who may require assistive devices.
  • Safely demonstrates use of mechanical lifts (Hoyer, Sara lifts).
  • Assists with admission, discharge, and transfer of residents.
  • Documents and records on facility E-MAR: vitals, bowel movements, meals, and skin concerns.
  • Reports any changes in condition to the Medication Technician, Nursing leadership, or the Assisted Living Manager.
  • Performs basic housekeeping for each resident, including laundry and trash.
  • Participates in staff meetings and staff development.
  • Uses time effectively on the floor to promote meaningful engagement.
  • Encourages activity engagement among team members, visitors, and residents.
  • Displays a positive, resident-focused demeanor with independence and dignity.
  • Performs other duties as assigned.

Description
POSITION SUMMARY:

The Certified Caregiver provides direct, hands-on assistance to residents in accordance with individualized service plans, supporting daily activities such as dressing, eating, toileting, mobility, and personal hygiene. Working collaboratively with medication technicians, nursing leadership, and the Assisted Living Manager, the caregiver monitors and documents resident status, promptly reporting changes in condition. This role also supports residents through safe use of mobility aids and mechanical lifts, basic housekeeping tasks, and active participation in activities that promote purpose and engagement. With a focus on dignity, independence, and compassionate service, the Certified Caregiver helps create a positive, person-centered living environment for all residents.

ESSENTIAL RESPONSIBLITY:

  • Documents and records on facility E-MAR: Vital signs, bowel movements, meal/snack percentages, and skin concerns.
  • Reports any discernable symptoms, reactions, or changes in condition to the Medication Technician, Nursing leadership, or the Assisted Living Manager.
  • Performs basic housekeeping services for each resident, including laundry and trash assistance.
  • Actively participates in regular staff meetings and staff development, as required.
  • Uses time effectively on the floor to promote meaningful engagement in care and activities.
  • Encourages activity engagement amongst team members, visitors, and residents to promote meaning and purpose.
  • Displays a positive, resident-focused demeanor that emphasizes independence, autonomy, and dignity.
  • Performs other duties or responsibilities as assigned by the Medication Technician, Nursing leadership, or the Assisted Living Manager.
EDUCATION/EXPERIENCE:
  • High school graduate or equivalent.
  • Verifiable, certified caregiver within the State of Arizona
  • Current fingerprint clearance card through the Arizona Department of Public Safety.
  • Current CPR and First Aid certification.
  • Memory care training, as required by the Department of Health Services (Or able to receive within 30 days of hire)

PHYSICAL DEMAND:
  • Able to sit, stand, bend, lift and move intermittently during working hours.
  • Can lift up to 40 pounds.
  • Can sustain prolonged standing and walking.

*All qualified applicants must successfully pass a background check, APS registry check, substance test, fingerprinting requirements, memory care training, and tuberculosis testing.
