Job summary

ALIYA Healthcare is seeking a detail-oriented and organized Central Intake Specialist to coordinate incoming referrals and support the admissions process across our skilled nursing facilities. This role serves as a central point of contact for referral sources, hospitals, discharge planners, and facility admissions teams, ensuring referrals are processed promptly and accurately.

Qualifications

• Experience in healthcare admissions or central intake preferred.
• Skilled nursing, long-term care, or post-acute healthcare experience preferred.
• Familiarity with hospital referrals, discharge planning, insurance verification, and admissions documentation preferred.
• Experience with PointClickCare (PCC) or similar healthcare software preferred.
• Strong communication, customer service, and organizational skills.
• Ability to handle confidential patient information.

Responsibilities

• Receive, review, and process incoming referrals for ALIYA Healthcare facilities.
• Respond promptly to referral inquiries from hospitals, case managers, social workers, discharge planners, and other referral sources.
• Review referral documentation for completeness and follow up to obtain missing information.
• Enter and maintain accurate referral and admissions information in designated systems.
• Coordinate with facility admissions teams and clinical leadership to determine referral status and next steps.
• Track referrals across facilities and communicate updates to the appropriate team members.
• Follow up on pending referrals to support timely decisions and prevent delays in the admissions process.
• Communicate facility availability, admission requirements, and referral status to referral sources as appropriate.
• Assist with identifying the most appropriate facility based on clinical needs, location, payer requirements, and facility capabilities.
• Maintain organized records of referrals, admissions, declined referrals, and follow-up activities.
• Monitor referral activity and provide updates or reports to leadership as needed.
• Build and maintain professional relationships with hospital partners and facility teams through timely, clear communication.
• Ensure referral information and patient records are handled confidentially and in accordance with HIPAA requirements.
• Identify opportunities to improve referral processing, communication, and intake workflows.
• Perform other duties as assigned.

Qualifications

• Previous experience in healthcare admissions, central intake, referral coordination, or a similar role preferred.
• Skilled nursing, long-term care, or post-acute healthcare experience strongly preferred.
• Familiarity with hospital referrals, discharge planning, insurance verification, and admissions documentation preferred.
• Experience with PointClickCare (PCC) or similar healthcare software preferred.
• Strong communication, customer service, and organizational skills.
• Excellent attention to detail and ability to manage multiple referrals and competing priorities.
• Ability to work efficiently in a fast-paced environment and meet time-sensitive deadlines.
• Proficiency with Microsoft Office and computer-based systems.
• Ability to maintain confidentiality when handling sensitive patient information.
