Job Description

Purpose of Position: Under the direction of the collaborative clinical team lead, performs clinical services related to the review, evaluation, and management of drug therapy for patients treated by physicians for specific diseases or disease states, in accordance with protocols and agreements.

Responsibilities:

• Operate under defined protocols with collaborating providers, adhering to NYC H+H and NYS guidelines.
• Implement and modify drug therapy plans for individual or groups of patients.
• Adjust drug management parameters and substitute authorized drugs as per protocols.
• Evaluate clinical lab tests related to drug therapy and order tests as authorized.
• Perform patient monitoring functions, including history collection, physical assessments, and vital signs checks.
• Document therapy modifications and communicate with other healthcare providers.
• Conduct medication reviews, reconcile medications, and resolve medication-related issues.
• Provide patient education on medication management and assist with medication access issues.
• Perform related duties as assigned.

Minimum Qualifications:

• Valid NY State pharmacy license and registration.
• Certification in Collaborative Drug Therapy Management (CDTM) by NYSED.
