PURPOSE OF POSITION
The clinical pharmacist provides patients and providers with therapeutic assessments and selected disease management, working with clinical staff, care managers and other clinical staff toward improving patient and practice outcomes.

SUMMARY OF DUTIES AND RESPONSIBILITIES
• Operates under defined protocols based upon agreement(s) with Collaborating Provider(s) and in accordance with the policies, procedures and protocols of NYC Health + Hospitals (H+H) and New York State (NYS) guidelines
• Know the contents of the NYC H+H Drug Formulary, inclusive of restricted agents and required ordering forms (e.g. the non-formulary drug request form).
• Be familiar with the standards of patient care; understand the medical conditions, treatment goals, treatment drugs & protocols.
• Assists physicians in creating and managing the drug regimens of patients with chronic disease states. This may include, but shall not be limited to, activities such as meeting with patients, adjusting medication dosages in concert with PCP, peak flow monitoring, and performing other services within scope of practice.
• Performs medicine reconciliation assessments as requested the providers and/or case managers to optimize the patient's drug regimen.
• Evaluates clinical laboratory tests related to the drug therapy management for the specific disease or disease state specified within the Protocol and orders such clinical laboratory tests, only if specifically authorized by the protocol and to the extent necessary to discharge their responsibilities under the law.
• Participates in activities related to improving outcomes for targeted disease management initiatives leading to improved health (such as COPD, Asthma, CHF, and Diabetes).
• Maintains accountability for department quality assurance, medication safety, drug use evaluation, adverse drug reaction monitoring, drug information resources, and staffing responsibilities as needed.
• Identifies patient/family gaps in knowledge of diagnosis and or treatment and development interventions with patient/family input to mitigate gaps.
• Documents modifications made to the patient drug therapy in each participating patient's record and notifies other treating providers, as required.
• Provides clinical pharmacy input during the preparation of interdisciplinary care plans for patients, assisting in the selection of the most cost effective, evidence-based regimen for a particular patient; monitor patients for appropriate lab and additional recommended follow-up.
• Orders and/or performs routine patient monitoring functions, as necessary, in drug therapy management including collecting and reviewing patient histories, conducting relevant physical assessments and ordering or checking of patient vital signs, including pulse, temperature, blood pressure and respiration, to the extent necessary to discharge responsibilities set forth in the Protocol.
• Monitors patient response to treatment, drug/drug and drug/disease interactions; assess medications prescribed at dosages higher that approved by the FDA for specific indications.
• Provides patient education on self-management and assists patients and providers with medication access issues to ensure understanding of the care plan.
• Work collaboratively and help coordinate activities with the care team to develop the patient care plan, promote adherence to treatment plan and encourage appropriate follow-up for optimal medical management.
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).

EDUCATIONAL LEVEL:
• Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
KNOWLEDGEABLE IN:
• Extensive knowledge of pharmacy and its related sub-disciplines, formulary development, drug use review, quality assurance, legal, regulatory, and standards of practice.
• Ability to research and analyze the medical literature including drug information, disease states, and clinical practice guidelines.
• Ability to integrate healthcare team.
• Excellent written and verbal communication, organizational skills, diplomacy, and professionalism
COMPUTER PROGRAMS/SOFTWARE OPERATED
• Proficiency in Microsoft Office Suite
YEARS OF EXPERIENCE:
2-3 years
