Responsibilities

• Complete assessments to determine eligibility for ongoing services.
• Maintain client plan of care that is reflective of needs as identified by interdisciplinary team.
• Develop crisis prevention plan and connect participants with additional community-based services to help prevent institutionalization.
• Process employee paperwork and coordinate training prior to service provision.
• Complete required documentation and training.
• Participate in and promote performance improvement activities.
• Work collaboratively with other region-wide programs to determine services offered in the Region.
