Prisma Health seeks a Clinical Documentation Integrity professional to review records and ensure accurate capture of severity of illness, mortality risk, LOS, and POA status. You will issue queries, reconcile records, and educate providers and teams as a second level reviewer.

Collaborate with Coding and other departments to guarantee precise documentation and code assignment, while staying current with ICD-10 and DRG guidelines.

Qualifikationen

• Bachelor's degree in nursing, physical therapy, respiratory therapy, health information management, or other healthcare related field.
• 4 years adult medical/surgical/critical care/ER/PACU nursing coding or related experience.
• 2 years clinical documentation improvement experience.
• MD/DO/PA/NP/CDDS/CDIP may substitute education/experience

Aufgaben

• Reviews medical records to identify documentation opportunities for severity of illness, risk of mortality, LOS, and POA status.
• Submits provider queries and reconciles discharged records.
• Collaborates with Coding and other departments to ensure accuracy and completeness of documentation and code assignment.
• Serves as second level reviewer, educator resource, and preceptor.
• Develops and maintains relationships with providers and care teams for education and follow-up.
• Keeps current with coding guidelines, treatment changes, and compliant query policies.
• Provides point of care education related to accurate documentation and POA status.
