Mercy is seeking a Clinical Documentation Specialist to perform concurrent and retrospective reviews of inpatient records. You will identify opportunities to improve documentation accuracy and ensure it reflects the true clinical scenario, including Severity of Illness and Risk of Mortality.

Collaboration with inpatient teams is essential to maintain high-quality documentation. The role requires RN/BSN or coding credentials with at least 1 year in acute care nursing, inpatient coding, or HIM,

Qualifications

• RN/BSN or coding/health IT credentials required or preferred.
• 1 year in acute care nursing, inpatient coding or HIM.
• Strong reading and writing communication; ability to work with diverse teams.

Responsibilities

• Conduct concurrent and retrospective reviews of inpatient records.
• Identify opportunities to improve documentation accuracy and completeness.
• Ensure documentation reflects true clinical scenario, SOI and ROM.
• Collaborate with inpatient care providers to ensure high-quality documentation.
• Maintain confidentiality and work within Mercy Service Standards.
