Qualifications

  • Four Year College Degree or High school diploma and three years experience in I&R services, Case Management, Call Center Services, Social Services.
  • Successful completion of Level II background screening is required.
  • Become Certified as an Information and Referral Specialist in Aging (CIRS-A).
  • Excellent written and verbal communication; proficient in Microsoft Office.
  • Ability to research services for older adults and prepare reports.
  • Ability to read and interpret regulations and policies.

Responsibilities

  • Conduct assessments and provide education to seniors seeking long-term care assistance.
  • Contact referred individuals for intake and screening.
  • Determine potential eligibility for services and resources.
  • Refer clients to other resources to meet their needs.
  • Post information in CIRTS enrollment screen for APCL as needed.
  • Review APCL lists and complete annual re-screenings.
  • Explain Medicaid eligibility criteria and process to applicants.
  • Refer to Medicaid Benefits Counselors for long-term care education.
  • Provide necessary forms at initial screening or annual re-screening.
  • Release individuals to Lead Agency or providers when funding is available.
  • Maintain accurate records using Refer software.
  • Provide emergency disaster assistance as directed.
  • Perform related tasks as assigned by supervisory staff.

This is a call center position. The Community Care for the Elderly (CCE) Intake Specialist conducts assessments, provides education and completes referrals for seniors 60+ living in the community, seeking long-term care assistance. This can include Medicaid and non-Medicaid funded programs.

Duties Responsibilities
  • Accepts referrals of clients from the ADRC Helpline and Department of Children and Families Adult Protective Services Unit as well as referrals from other organizations and individuals.
  • Contact the referred individuals for intake and screening, administering the standardized screening instrument to obtain necessary and essential information to determine the individual’s needs for services.
  • Determine potential eligibility for DOEA and non-DOEA funded programs.
  • Direct individuals to other non-DOEA resources, as appropriate, to have their needs met.
  • Post information in the CIRTS (Client Information and Registration and Tracking System) enrollment screen related to program APCL, as needed.
  • Contact individuals on the Assessed Priority Consumer List (APCL) as required, to review any reported changes in their situation and complete annual re-screenings to ensure prioritization for services.
  • Explain Medicaid eligibility criteria and eligibility process to prospective applicants.
  • Refer individuals to the ADRC Medicaid Benefits Counselors and Medicaid Waiver Specialists for Long-Term Care Education.
  • Provide all necessary and required forms, at the time of initial screening or annual re-screening.
  • Release individuals to the appropriate Lead Agency or service provider for CCE, HCE and ADI services, when funding is available.
  • Collects and maintains accurate records of caller requests using the Refer computer software for appropriate documenting and reporting.
  • Provides emergency disaster assistance and staff support during an emergency disaster event as directed by designated staff.
  • Performs related tasks as assigned by supervisory staff and Senior Management.

Requirements
  • Four Year College Degree or High school diploma and three years experience in I&R services, Case Management, Call Center Services, Social Services; or related work.
  • Bilingual (English-Spanish) preferred.
  • Successful completion of Level II background screening is required. Become Certified as an Information and Referral Specialist in Aging (CIRS-A) through the Alliance of Information and Referral Systems within one year of employment.
  • Excellent written and verbal communication skills.
  • Proficiency in Microsoft Office: Word, Excel, PowerPoint, Outlook (required).
  • Ability to research topics related to services for older adults (funding, program design, etc.), analyze data, and provide written and/or oral reports as required.
  • Ability to read, analyze and interpret technical procedures, governmental and non-governmental regulations, and policies.
  • Ability to add, subtract, multiply, and divide all units of measurement using whole numbers, common fractions, and decimals.
  • Ability to complete and receive a passing score in Uniform Client Assessment Training within 90 days of employment.
  • Work Environment and Physical Demands
  • The incumbent generally works in an office or home office environment, uses computers, telephone, and other office equipment as needed to perform duties. The noise level in the environment is usually moderate.
  • Sedentary work primarily involves sitting and standing. Required to use repetitive hand motion, talk, handle, reach, hear, bend or lift up to 20 pounds.
