Responsibilities

• Coordinate transplant/VAD referrals and evaluate candidate eligibility.
• Advocate for patients and families throughout the transplant journey.
• Ensure compliance with UNOS regulations and maintain listing status.
• Build relationships with referring physicians and care teams.
• Provide education and outreach on transplant/VAD services.
• Mentor staff and participate in staff development.
• Develop patient education plans and coordinate ongoing assessments.
• Assist with peri-operative care coordination and post-transplant follow-up.
