Qualifications

• Current WI RN licensure (s.441.06)
• Experience with personal care services
• Orientation and competency assessments per state law
• Valid WI driver's license when required
• Proof of auto insurance when required
• Able to pass WI caregiver background checks

Responsibilities

• Plan, coordinate, and evaluate Member’s care
• Ensure regulatory and nursing compliance
• Conduct client assessments and transitions
• Obtain and renew physician orders
• Develop and update care plans
• Report significant condition changes
• Develop service reporting systems
• Train and instruct care staff
• Supervise, delegate, and evaluate staff
• Document consultations and nursing care
• Promote client independence and education
• Participate in employee health screenings and administering TBs
• Complete required PC documentation
